Clinical outcomes and management of contrast hypersensitivity in patients requiring repeated computed tomography imaging

Introduction: In collaboration with the Department of Rheumatology, Allergy and Immunology, our study aims to review outcomes propose an improved workflow for management patients prior hypersensitivity reactions iodinated contrast media (ICM). Method: Outpatients coming contrast-enhanced computed tomography (CECT) were stratified into 3 categories (definite, unconfirmed inaccurate) based on likelihood their label. Patients could be offered a different ICM, receive same or referred allergist further evaluation. There 4 outcomes: (1) alternative ICM tolerated; (2) tolerated again; (3) patient developed reaction either original ICM; (4) CECT was deferred until assessment by allergist. Comparison made pre- post-intervention see if improved. Results: 132 who total 154 visits (90.3% had documented hypersensitivity). Post-intervention, number postponed premedication decreased (81.0% 34.7%). reduction in (from 42.9% 14.3%). Of 12 assessed allergist, 6 continue using advised abstain from administration 2 pending testing third agent. Conclusion: Active intervention radiologist can decrease postponed, converted cancelled studies as well reduce adverse allergic-like events.
